Dual eligible beneficiaries and Medi-Cal seniors and persons with disabilities are among California’s highest-need residents in the facility setting. They tend to have many chronic health conditions and need a complex range of medical services that have been mandated by the State and Federal regulations. Diabetes, Peripheral Vascular Disease, and Neuropathy are widespread among this “at risk” population and continued regular evaluation and treatment of common conditions are imperative to keeping these patients from ending up in a hospital emergency room for something that could have been easily prevented.

Our physician take care of thousands of California facility based patients each year in coordination with the entire healthcare team. If we see a problem that is too difficult or impractical to treat in the facility setting we simply alert the primary care physician and utilize the seamless managed care system already in place. We believe functioning within this framework greatly decreases the fragmentation of services and costs, while improving accountability for health outcomes creating a healthier, happy patient.
